Full Body Interment Requirements

Original Copy of CERTIFICATE OF RIGHTS
. Two (2) VALID government-issued IDs OF Lot Owner
SENIOR ID or PWD ID OF DECEASED, if applicable
CERTIFIED TRUE COPY OF DEATH CERTIFICATE
BURIAL PERMIT ISSUED BY MUNICIPALITY
CASKET MEASUREMENT provided by the servicing funeral home
If Lot Owner is not personally scheduling the interment,
- AUTHORIZATION LETTER OF Lot OWNER allowing HIS/HER REPRESENTATIVE TO SIGN and Transact the INTERMENT
- Photocopy or scanned copies of two (2) VALID government-issued IDs OF LOT OWNER (PHOTOCOPY/SCANNED COPY W/ 3 SPECIMEN SIGNATURES)
- Photocopy or scanned copies of two (2) VALID government-issued IDs OF AUTHORIZED REPRESENTATIVE
If lot owner is the deceased or is already deceased, EXTRAJUDICIAL SETTLEMENT or Undertaking for submission of Extrajudicial settlement signed by the immediate heirs

Bone Interment Requirements

EXHUMATION RECEIPTS
BURIAL PERMIT – (PER BONES)

Ash Interment Requirements

Original copy of CERTIFICATE OF RIGHTS
Two (2) VALID government-issued IDs OF Lot Owner
CREMATION CERTIFICATE
BURIAL PERMIT

Other Interment Guidelines

Client must notify the company for any interment at least seventy-two (72) hours prior to the scheduled interment. Likewise, the necessary interment fee and required payments for the lot/s shall be paid. The company reserves the right to reschedule, postpone, delay or deny interment depending on capacity to inter or availability of personnel and equipment.
Burial Permit/Tax required by law shall be on the account and responsibility of the client. In no case shall interment be allowed without the necessary burial permit. (No Burial Permit – No Interment)
Lawn lots and Mausoleum lots must be fully paid before interment will be scheduled.
Change of interment schedule and cancellation of interment shall be subjected to applicable fees.
All interment shall be permanent, however transfer of remains/vaults may be allowed when deemed necessary and considered valid by the company.
